Gujarat Birth and Death Certificate Name Correction: New Advisory 2026

The Office of the Additional Director (Statistics), Health, Medical Services and Medical Education, Gujarat, has issued an important Advisory regarding correction, modification and addition of names in Birth and Death Certificates.

The new Advisory, approved on 2 September 2026, cancels the earlier Advisory dated 25 November 2025 and provides clearer guidelines for Registrars and Sub-Registrars.

Important Rules for Name Correction

1. Father’s Name in Birth Certificate

The biological father’s name cannot be replaced with the mother’s name in the father’s name column. The father’s name has to be recorded in the relevant column.

2. Mother’s Name in the Child’s Full Name

If both parents are living with the child, the mother’s name and surname can be included in the child’s full name on the basis of supporting documents and the father’s written consent.

3. Divorce and Child Custody Cases

If a court order gives custody of the child to the mother, the child’s name can include the mother’s name and surname when the court order clearly permits or specifies such a change.

4. Changing the Order of the Name

The name in a Birth Certificate can be modified to include the surname first, followed by the child’s name and father’s name, subject to appropriate supporting documents.

5. Father’s Name as Middle Name

Parents who do not wish to have the father’s name as the child’s middle name may keep it optional.

6. Death Certificate

In a Death Certificate, the father’s or husband’s name as a middle name may also be kept optional.

7. Father’s Name and Surname

If the applicant does not want the father’s name or surname after the child’s name, both may be kept optional according to the applicable procedure.

8. Name Changes in Death Certificate

Similarly, the father/husband’s name and surname after the deceased person’s name may be kept optional.

9. Different Surname or Correction of Child’s Name

Corrections can be made on the basis of relevant supporting documents, including the Government Gazette, corrected name documents, Government Photo ID such as Aadhaar or PAN Card, and other necessary documents.

Is an Affidavit Required?

An important clarification in the Advisory is that an affidavit is not required for the above matters. The correction is to be made on the basis of self-attested supporting documents.
